Diploma Name Change Request

The name that WSU uses as your official name can by found on your Unofficial Transcript.

Change your first and/or middle name or add an accent

Use this form to change your first and middle name printed on the diploma(s) or add an accent to your name. These changes ONLY apply to the diploma, and the commencement program (if submitted prior to the graduation application deadline), and will not be reflected in the transcript.

To change your last name

Contact Payroll Services to change your last name on your diploma, transcript, or any official WSU documents. Small changes to a last name, or the way it is presented or accented, may be requested by sending a message to wsu.graduation@wsu.edu - changes to the last name made in this way will only affect the way the last name is displayed on the diploma.

International Programs Students

Your diploma name must match the name on your VISA.

Students intending to work abroad

Your diploma name must match your passport for VISA applications.
