Change your first and/or middle name or add an accent
Use this form to change your first and middle name printed on the diploma(s) or add an accent to your name. These changes ONLY apply to the diploma, and the commencement program (if submitted prior to the graduation application deadline), and will not be reflected in the transcript.

Contact Payroll Services to change your last name on your diploma, transcript, or any official WSU documents.
